A new hearing request form, along with many other forms you may find helpful, can be obtained from the Driver License section of the Washington State Department of Licensing's website at: http://www.dol.wa.gov/driverslicense/
There is a $200 fee for requesting a DUI hearing. This fee will not be refunded if you cancel the hearing or fail to appear when scheduled.
